Why do I need to provide my Tax Identification (TIN) number to register?

CMS requests that plans keep records of providers trained. Your Tax Identification is used to accurately identify and validate what provider(s) is (are) credited with having completed the Model of Care training module. If the TIN you provide is not on file, you will be unable to access the course.
