Instructions
- Before you begin, make sure that you have the CE “voucher codes” that were announced at the beginning of each session. You may have recorded the codes in your syllabus.
- There are two links on the left-hand side of this webpage: (a) If you are visiting this site for the first time, click on the “Register” link. (b) If you have already created a login and password, please click on the “Login” link.
(Note: If you used this online service to complete program evaluations for Outlook 2008 and/or Outlook 2009, your login/password are still valid for Outlook 2010. If you cannot remember the login and/or password you created for this online service, please contact Shannon Presidio at (916) 779-1410, ext. 324 or spresidio@pharmacyfoundation.org.)
- On your Personal Page, enter the session’s “voucher code” in the designated area and follow the on-screen instructions to complete the evaluation.
- Upon successful completion of the evaluation, your Statement of Credit will be available from the “History” link located on your Personal Page. Click on the “Print” link to access your Statement of Credit. (Note: You will need to print out a separate Statement of Credit for each session attended).

| On-line evaluations must be completed by Monday, March 15, 2010. Evaluations submitted through the on-line process after this date may not be valid for CE credit. |

The Pharmacy Foundation’s CE Policy & Accreditation Statement
The minimum credit that may be awarded for any program is one contact hour (60 minutes of live programming). Attendees must complete an evaluation for each program they attend in order to receive a Statement of Continuing Pharmacy Education Credit. Failure to complete and submit program evaluations may result in the forfeiture of CE credit.

The Pharmacy Foundation of California (#113) is approved by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. A Statement of Continuing Pharmacy Education Credit will be provided within six weeks after the event to paid registrants who actively participate in the entire program and complete an evaluation form by March 15, 2010. All courses valid for CE credit up to 3 years from date of initial presentation. |
The Board of Registered Nursing accepts many ACPE accredited courses for licensure renewal and the Pharmacy Technician Certification Board accepts many ACPE accredited courses for Technician Certification renewal. However, nurses and technicians are responsible for contacting their respective boards to verify the eligibility of the units for license or certification renewal.
